Conditions of Loan
- The equipment is for the benefit only of the patient/client to whom it is issued whilst they have a clinical or nursing need for the same.
- The equipment must not be removed out of the Sheffield area without the permission of SCELS.
- The equipment must not be willfully damaged, adapted or changed in any manner.
- If any defect or irregularity is found in the condition of equipment, this must be reported to SCELS on 0114 226 3800 as soon as possible. On no account must
any repair or modification be undertaken by anyone other than authorised personnel.
- Access must be permitted at all reasonable times for equipment to be inspected and/or maintained or for collection of equipment by SCELS in accordance with
these conditions of loan.
- Patients/clients must ensure that equipment on loan is covered by their household insurance and SCELS reserve the right, in appropriate cases to request sight of
documentary evidence of appropriate insurance.
- Equipment loan is for an initial period of three months. Any extension of the loan period [subject to continuing clinical or nursing need] can be sought by contacting SCELS.
- If the equipment supplied is a bed for use in the patient's/client's home, Statutory Regulations, which restrict the use of gas appliances in bedrooms may apply. In
providing the equipment, SCELS gives no warranty as to the fitness of any gas or other appliances at the patient's/ client's home and it remains the obligation of the patient/client to ensure that ant Statutory Regulations relating to the structure or use of premises at which the equipment is installed are complied with.
- Equipment no longer required should be returned, as soon as possible, whether or not the loan period has expired, and must not be given or loaned to any other person.
