How do I requisition equipment?
Requisitioning [GP's, Professional Therapists and District Nurses only].
Before requisitioning you will need to log in with your allocated code. When you have logged in you will have your log in code acceptance confirmed on screen.
All equipment codes starting with an "H" are accessible by any Health Care Professional. Stock codes starting with an "S" are accessible by any Social Services Occupational Therapist plus Community Occupational Therapist and District Nurses who have received the relevant training and authorisation from Social Services.
When requisitioning equipment:
[those professionals authorised to requisition the item[s] are noted in red type on the top of each equipment page]
Each item requires you to complete the "compulsory fields" - tick boxes- within the grey area. This covers the method of supply, how quickly the equipment is required and the quantity required. If you have failed to complete this area correctly you will be advised on screen
When you have completed the item request click order. You will then see on screen confirmation of your request on the Requisition Form, click OK and the item will appear on the Requisition Form. You will also see "Patient Details" on screen. These should be completed when you have finished ordering. If further items are required click continue ordering at the bottom left hand side of the screen. You can see your order at any time by clicking on "View Order" in the TOP menu bar. At that stage you also have the option of cancelling all items if you click on clear order or individual items if you tick delete to the left hand side of the item, on the Requisition Form.
On completion of your request/order click on view order on the top panel of the screen [right hand side]. The item[s] code and other completed item details will automatically appear on your Requisition Form.

How is the equipment shown?
You can view items within a section as "thumbnail" photographs along with the code and then by clicking onto the "thumbnail" of a specific item reveal a larger photograph along with the item's description and details of those Professional Referrers authorised to requisition the item[s].

Can anyone order/requisition equipment?
No, only GP's, Professional Therapists, or District Nurses. Patients/Clients and Carers cannot requisition equipment and should refer to their GP, Professional Therapist or District Nurse for assessment.

How do I obtain a short-term wheelchair on loan?
We have a limited number of wheelchairs available on medical referral. Short-term loan of wheelchairs is available from the Red Cross.
